The Adult Community Health Service includes Chronic Conditions Management, Community Rehabilitation, Post-acute, Rapid Response Falls Service and Persistent Pain Management service. The multidisciplinary team works closely with acute services and Primary Care including General Practice and many other non-Government services to enable clients to better self-manage their health.
About You:
- Contribute to an innovative evidence-based model of health care for aged care clients with complex health care needs.
- Deliver quality, patient focused assessment, intervention and case management, utilising specialist contemporary knowledge and clinical experience, for the management of the frail and elderly across the care continuum, with the primary focus of establishing sustainable supports for the prevention of functional decline and avoidance of hospital admission and re-admissions.
- Deliver high level discipline specific clinical and consultancy services across all streams of the Adult Community Health Service. Recognised expertise, knowledge and skills are obtained through significant professional development activities, post-graduate education or formal qualification(s).
- Utilises evidence-based practice and professional judgement for a complex clinical caseload, to facilitate novel multidisciplinary clinical decisions and management with minimal clinical practice supervision.
