About the Team

In March 2017, QCH and the Paediatric Trauma Service were successful in achieving full trauma verification as a Level 1 Major Trauma Centre. We are the first paediatric hospital in Australasia to receive this verification status, recognising the skills and facilities for patients presenting to QCH post traumatic injury.

The Trauma Service is a specialised team of healthcare professionals that are very experienced in trauma care and management. Although we have nominated 6 areas of defined responsibility, injury prevention, clinical activities, education, research and data collection, quality management and networking and administration. Our service has focussed on 3 key areas at QCH, Service, Teaching and Research.

The trauma service team is a small dynamic and cohesive multi-disciplinary team that takes a leadership role in injury prevention, trauma education, quality management and data collection and research. We continuously drive innovation and improvements in child and family centered trauma care.

About the Role

To provide advanced trauma clinical and professional practice, facilitating a safe and efficient patient journey across the continuum of care through applying the Nurse Navigator principles of coordinating patient centred care, creating partnerships, improving patient outcomes, and facilitating systems improvement. The role improves complex patient care, increases system efficiencies, reduces costs by streamlining and coordinating care.

The Nurse Navigator (NN) Trauma Family-Support is a Registered Nurse accountable at an advanced practice level for the provision of specialised knowledge and advanced clinical practices supporting patients and families requiring admission follow major trauma.

The Nurse Navigator Trauma Family-Support is involved throughout the patient journey commencing from admission and up to two years post discharge. The successful applicant will provide support, leadership and direction to nursing staff, students and the multi-disciplinary team as required. The position provides collaborative family-centred care whilst promoting safe and professional standards of nursing practice and conduct. The Nurse Navigator Trauma Family-Support will work closely and in collaboration with the Trauma Social Worker, the Trauma Nurse Manager and the Director of Trauma.

The Nurse Navigator Trauma Family-Support role will work collegially and collaboratively with a range of internal and external providers and organisations to facilitate a patient centred journey within the current (and future) complex health care systems. This role aims to lead and facilitate a safe and efficient patient journey across the care continuum through the reduction of service fragmentation, educating and empowering individuals and families and coordinating patient care.  The NN will also work collaboratively with other NN's in CHQ and across the state to provide safe, quality and cost-effective family-centred paediatric nursing care that is consistent with contemporary standards.
