About the role:
- Accountable for advanced level, autonomous clinical judgement in advising treating teams regarding patient episode of care changes for sub and non-acute patients (SNAP), complex transition patients and discharge planning needs.
- Deliver leadership, guidance, and stakeholder support across SNAP, Aged Care, NDIS, and community services to coordinate complex patient transitions.
- Build service capacity across CQHHS by providing strategic and clinical direction to improve systems, processes, and outcomes for patients with complex discharge needs.
- Apply specialised expertise in NDIS, disability, legislation, functional assessment, equipment prescription, and home modifications to support clinicians, senior management, and key stakeholders.
About you:
- Possession of an Allied Health qualification from a registered tertiary institution in the disciplines of Occupational Therapy, and/or Social Work.
- Registration with Australian Health Practitioner Regulation Agency (AHPRA) with a current annual practicing certificate is essential, or for members of non-registered and self-regulated professions, eligibility for professional membership of the relevant professional association.
- While not mandatory, a postgraduate qualification relevant to the role would be well regarded.
What we offer:
Total remuneration value of up to $14,385 p.m is comprised of:
- An hourly rate between $73.12 - $76.30 p.h.
- 12.75% employer superannuation contribution
