Your new role:
As the Clinical Nurse Consultant – Clinical Care Coordinator, you will provide expert clinical leadership and consultancy to support the delivery of high-quality mental health care across Metro South Health. Working closely with multidisciplinary teams and community partners, you will help ensure seamless care pathways for consumers accessing The Way Back Support Service.
Key Responsibilities
- Provide advanced clinical leadership and specialist mental health nursing expertise to support safe, recovery-focused care.
- Coordinate referrals, care pathways and service integration between Metro South Health and The Way Back Support Service.
- Lead quality improvement, education and clinical governance initiatives to enhance service delivery and patient outcomes.
- Build strong partnerships with multidisciplinary teams, consumers, carers and external stakeholders to support continuity of care.
About you:
You are an experienced and compassionate mental health nurse who is passionate about improving outcomes for people experiencing suicidal crisis and complex psychosocial needs.
What You'll Bring
- Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia (NMBA).
- Advanced clinical knowledge and experience in mental health nursing, including risk assessment, care coordination and recovery-oriented practice.
- Exceptional communication, consultation and stakeholder engagement skills, with the ability to influence and lead across services.
- Proven ability to drive quality improvement, support clinical governance and provide education and mentorship to others.
