About the role
Provide advanced clinical nursing care within the Hospital in the Home (HITH) service, with a key focus on coordinating the Acute Patient Assessment of Care in the Home Environment (APACHE) program. This role supports delivery of hospital-level care in community settings, including patient homes, clinics, and outreach locations, ensuring timely assessment, triage, and management of patients requiring acute and sub-acute care outside the traditional inpatient environment.
As a Clinical Nurse, you will work with a high degree of autonomy while providing clinical leadership, coordination, and support to nursing staff and the multidisciplinary team. You will play a central role in managing referrals and patient flow across the service, applying advanced clinical judgement, communication, and decision-making skills to deliver safe, efficient, and person-centred care across diverse settings.
