About the role
The goal-oriented, low intensity rehabilitation transition care program is provided in either the person's home or in residential facility setting or both over the duration on the program. The Clinical Nurse works autonomously, though collaboratively within the Nursing team and the broader Multi-disciplinary team. The Clinical Nurse is responsible for delivering nursing care and providing Case Management whilst clients are on the program.
About you
Our ideal candidate will be someone who can demonstrate the following:
- Demonstrated advanced clinical knowledge in the area of Aged and Community care delivering expert nursing care in complex situations while providing support and direction to other team members.
- Demonstrated ability to role model excellent standards of comprehensive, evidence-based nursing care and documentation to facilitate quality patient outcomes, in accordance with National Safety and Quality Health Service and professional standards.
- Ability to prioritise clinical and related administrative duties, including effective and efficient planning and organisation of complex caseloads with proven ability to use information and reporting systems appropriate to the position.
- Demonstrates honesty, integrity and respect for all patients, carers and staff.
- Demonstrated ability to communicate effectively with members of the healthcare team, patients, relatives and visitors and maintain a positive workplace culture through well developed consultation, liaison, negotiation and conflict resolution skills.
Mandatory Requirements:
- Appointment to this position requires proof of qualification and registration or membership with the Australian Health Practitioner Regulation Agency.
- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
