About us:

Adult Community Health (ACH) is a multidisciplinary team which is responsible for providing high quality services across the continuum of care to client/patient groups within the Cairns area from Fishery Falls in the South to Ellis Beach in the North and Kuranda to the West. Our focus is to provide a wraparound service to the acute sector to reduce admissions, support timely discharge and encourage self-management in the community with a focus on effective links to Queensland Ambulance Service, hospital, Primary and community care providers.

We're committed to improving the health and wellbeing of people in Far North Queensland by providing high quality acute health care services.
