About the role
Provide advanced clinical nursing care within the Home Hospital, Community and Oral Health service, specialising in Acute Wound and Stomal Therapy. This role focuses on delivering expert wound and stoma management for patients across community and sub-acute settings, ensuring high-quality, evidence-based care and optimal patient outcomes.
You will work with a high degree of autonomy while supporting clinicians and nursing staff in complex care situations, contributing to care coordination, education, and service delivery across the Post Acute Care Service. The role also involves promoting best practice, supporting patient self-management, and participating in quality improvement and clinical development activities within a multidisciplinary team environment.
