About the role
Provide advanced clinical nursing care within the Hospital in the Home (HITH) service at Metro North Health, delivering high-quality, patient-centred care to individuals in their home or community setting. This role focuses on providing expert assessment, care planning, and clinical management for a diverse patient cohort, supporting hospital-level care outside traditional inpatient environments.
You will operate with a high degree of autonomy while supporting nursing staff and contributing to care coordination, clinical leadership, and service delivery. The role involves responding to complex clinical situations, promoting patient independence through education, and actively participating in quality improvement and multidisciplinary care initiatives.
