About the role

This is a multi-stream leadership role overseeing chronic disease programs across 11 Cape York communities, focused on improving prevention, management and health outcomes in some of Queensland's most remote regions.

For Health Practitioners

Leads and manages multidisciplinary clinical programs focused on primary, secondary and tertiary chronic disease prevention and management across 11 Cape York communities. The role oversees advanced diabetes education, Nurse Practitioner Chronic Disease services, and the Cape York Kidney Team. Services are delivered through outreach clinics, telehealth, and community-based care in partnership with Aboriginal Health Workers and local health services, with a strong focus on culturally safe care, chronic disease strategies, health promotion and performance monitoring.

For Nurses

Leads clinical teams delivering health promotion and chronic disease management at a primary health care level across Cape York communities. The portfolio includes diabetes, kidney care and chronic disease programs, with a focus on healthy lifestyle initiatives such as nutrition, physical activity, prevention and self-management support.
