About us:
The North Continuing Care Team provides specialist multidisciplinary assessment, treatment and care for patients who experience a wide range of severe and persistent psychosis, mood, anxiety, behaviour and eating disorders that significantly impact daily functioning and quality of life. Our services are provided across the community to ensure continuity of care.
We're committed to improving the health and wellbeing of people in Far North Queensland by providing high quality acute health care services.
About your opportunity:
- Using a biopsychosocial model conduct mental health assessments and assist consumers, families and supports to develop a comprehensive individual recovery plan and facilitate the effective delivery, monitoring and evaluation of this plan.
- Work with families and communities to promote recovery and community inclusion and to access generic supports, opportunities and resources.
- Develop and maintain collaborative work practices and relationships with other agencies (including non-government organisations), service providers and consumer and carer groups to ensure consumers have access to an integrated service system.
- Effectively communicate at all levels across the organisation and with external stakeholders, including non-government organisations, consumers and their carers.
- Provide clinical education and supervision pertaining to routine tasks for students, those involved in observational placements and less experienced clinicians with the support of senior clinical staff.
