About Your Role
As our Clinical Nurse (HOMES – Child and Adolescent), you'll deliver advanced nursing care and holistic case management to Aboriginal and Torres Strait Islander children, young people and families living with complex health needs.
In this role you will:
- Coordinate and deliver advanced nursing care and case management
- Provide home visits and outreach services across Metro South Health
- Build trusted relationships with children, young people and families
- Develop holistic care plans that support self-management and wellbeing
- Coordinate care across health, community and social support services
- Promote culturally safe, evidence-based nursing practice
About You
You are an experienced Registered Nurse who is passionate about improving the health and wellbeing of Aboriginal and Torres Strait Islander children, adolescents and families.
You'll also bring:
- Demonstrated experience managing complex chronic conditions
- Strong case management and care coordination skills
- Experience working within Aboriginal and Torres Strait Islander health
- Excellent communication and relationship-building abilities
- A collaborative approach focused on health equity and community partnerships

About Southern Queensland Centre of Excellence, Inala Indigenous Health Service
The Southern Queensland Centre of Excellence (SQCoE) for Aboriginal and Torres Strait Islander Primary Health Care forms part of the Directorate and is based at Inala providing the following services:
- Inala Indigenous Health Service: provides GP, nursing and allied health clinical services; Specialists clinics; and Mums and Bubs program.
- Integrated Mental Health and Substance Use Services: provides culturally appropriate service for Aboriginal and Torres Strait Islander people, supporting and enhancing patient centred care. The aim of the service includes reduction in hospital admission rates; reduction of mental health distress and increased support for Aboriginal and Torres Strait Islander clients to navigate community resources to address social determinants of health. The service model incorporates outreach either in the client's home or elsewhere as identified by the client.
- The SQCoE works closely with Elders and community representatives and undertakes research projects with the aim to improve health outcomes for First Nations Peoples.
