The Nurse Navigator Services team delivers flexible, patient-centred care for adults living with chronic and complex health conditions. Working across multidiciplinary teams and healthcare settings, the service coordinates integrated care, improves access to healthcare, reduces fragmentation and supports better health outcomes across THHS.

Competitive salary and benefits:

Generous remuneration package value up to $148,425 p.a. comprising:

  • Salary range of $70.58 to $76.63 p.h. (NG7)
  • 4 to 6 weeks annual leave with 17.5% loading
  • 12.75% employer contribution to superannuation
  • Salary Sacrificing may be available

About your opportunity:

Join the Nurse Navigator Services team as a Nurse Navigator (Complex Chronic Diseases - Adult). In this advanced practice role, you will provide patient-centred care for adults living with chronic and complex conditions, working collaboratively with multidisciplinary teams to improve healthcare access, streamline patient journeys and achieve better health outcomes through intergrated care.

Requirements:

  • Mandatory qualifications (Bachelor of Nursing or equivalent) and proof of registration (AHPRA).
  • This is a Vaccine Preventable Disease (VPD) risk role.
  • Probation period may apply.
  • Pre-employment checks will be conducted.
