Your new role:

Provide comprehensive community-based mental health assessment, intervention and case management for older adults, working collaboratively within an experienced multidisciplinary team. You'll deliver high-quality, recovery-focused care, support patients and their families through every stage of their journey, contribute to service improvement and research initiatives, and help educate the next generation of health professionals while continuing to grow your own clinical skills.

About you:

You are a compassionate and motivated allied health professional who is passionate about delivering high-quality, recovery-focused mental health care to older adults while thriving in a collaborative multidisciplinary team. You will be able to:

  • Demonstrate experience in delivering evidence-based mental health assessment, intervention and case management with a strong patient-centred focus.
  • Communicate confidently and build effective relationships with consumers, families, carers and multidisciplinary teams to achieve the best outcomes.
  • Manage a varied clinical caseload with sound clinical judgement, strong organisational skills and accurate documentation.
  • Contribute to clinical education, quality improvement and service development while embracing ongoing learning and professional growth.
  • Champion safe, inclusive and compassionate care, promoting dignity, respect and positive health outcomes for every consumer.
