Your new role:

Physiotherapist, where you'll provide expert community-based physiotherapy services for people requiring restorative care and rehabilitation. Working with a high level of clinical autonomy, you'll lead evidence-based practice, mentor and develop clinicians, drive service innovation and quality improvement, and collaborate across multidisciplinary teams to deliver exceptional patient-centred care that helps people achieve their goals safely at home. 

About you:

You are an experienced and passionate Physiotherapist with advanced expertise in community-based rehabilitation and restorative care. You thrive in a collaborative environment, provide exceptional patient-centred care, and are driven to lead innovation, mentor others and improve health outcomes for the communities you serve. You'll be successful in this role if you have:

  • Advanced clinical expertise in community-based physiotherapy, restorative care and rehabilitation, with the ability to independently manage complex clinical presentations.
  • A collaborative leadership style with experience mentoring, supervising and developing physiotherapists, allied health assistants and students.
  • Exceptional communication, stakeholder engagement and clinical decision-making skills, with the ability to build strong multidisciplinary partnerships.
  • A passion for service improvement, evidence-based practice and innovation, with experience leading quality improvement, research or new models of care.
  • A commitment to delivering safe, compassionate and person-centred care while contributing to a positive, inclusive and high-performing team culture.

About Metro South Health@Home

Metro South Health@Home delivers innovative, person-centred healthcare services that support consumers to receive high-quality care closer to home. Through multidisciplinary teams including Rehab@Home, Acute Care@Home and community-based services, we provide rehabilitation, restorative care, hospital avoidance and hospital-in-the-home programs that help people maintain independence, improve wellbeing and achieve their health goals.

The Rehab@Home service provides time-limited, goal-focused rehabilitation and restorative care for clients, including the Transition Care Program (TCP), supporting older people to maximise their functional capacity and transition safely within community or residential settings.

As part of Metro South Health's Community and Oral Health Directorate, our teams work collaboratively with consumers, families, primary care providers and healthcare professionals to deliver safe, equitable and innovative models of care that improve outcomes for our diverse communities.
