About the role

As a Clinical Nurse Consultant - Cardiology (Interhospital Transfer), you will provide advanced clinical leadership, specialist expertise and coordination to support the safe, timely and efficient transfer of cardiology patients between hospitals. Working within the Heart Lung Program at The Prince Charles Hospital, you will undertake comprehensive clinical assessment and triage of interhospital referrals, ensuring patients receive appropriate access to specialist cardiac services.

This role works closely with multidisciplinary teams and internal and external stakeholders to coordinate patient transfers, support clinical decision-making, streamline referral processes and promote high standards of patient safety, quality improvement and clinical governance. You will act as a specialist clinical resource while contributing to service development, innovation and improved patient outcomes across the Cardiology Program.

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

  • Demonstrate advanced specialist knowledge, clinical expertise and sound judgement in Cardiology and interhospital referral management.
  • Conduct comprehensive clinical assessment, triage and prioritisation of interhospital Cardiology referrals to support safe and timely patient transfers.
  • Provide expert clinical leadership, advice, mentoring and support to multidisciplinary teams involved in cardiology transfer coordination.
  • Collaborate with healthcare professionals and stakeholders to improve referral pathways, patient flow, communication and continuity of care.
  • Support quality improvement, innovation and policy implementation activities that strengthen clinical governance and enhance patient outcomes.
