About the role

Help shape the future of nurse-led care for older people across The Prince Charles Hospital's Interim Care and Cognitive Impairment Services. This advanced practice opportunity is suited to an endorsed Nurse Practitioner who is passionate about improving outcomes for older adults living with cognitive impairment, dementia, delirium and other geriatric syndromes through innovative, evidence-based models of care.

Working across hospital and residential aged care settings, you will provide autonomous clinical assessment, diagnosis, care planning and treatment within an approved Scope of Clinical Practice. A key focus of the role is leading and supporting contemporary nurse-led models of care, partnering with multidisciplinary teams and community providers, and contributing to improved health outcomes for older people with complex care needs.

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

  • Provide advanced clinical assessment, diagnosis, treatment and ongoing management for older people with geriatric and cognitive impairment conditions within an endorsed Nurse Practitioner scope of practice.
  • Demonstrate autonomous decision-making and advanced clinical expertise while delivering evidence-based care across hospital and residential aged care environments.
  • Lead and support nurse-led models of care that enhance patient outcomes and improve access to specialised gerontology and cognitive impairment services.
  • Build strong partnerships with multidisciplinary teams, residential aged care providers, consumers and carers to support coordinated and person-centred care.
  • Contribute to research, quality improvement, education and clinical leadership activities, including mentorship and support for Nurse Practitioner candidates and other clinicians.
