About the role

Join the North West Hospital and Health Service Cardiac Services team as a Clinical Nurse - Cardiac and Respiratory Rehabilitation and help deliver innovative, patient-centred care closer to home. Working within a collaborative multidisciplinary team, you will provide specialist clinical care, education and rehabilitation for patients across the North West, while contributing to service improvement and the development of contemporary models of care.

  • Lead and coordinate cardiac and respiratory rehabilitation, including patient assessment, care planning, education, medication management and clinical review.
  • Deliver care closer to home, supporting community-based programs, outreach and integrated services across the North West Hospital and Health Service catchment.
  • Provide clinical leadership and expertise, working autonomously while supporting the broader Cardiac Services team and contributing to education, mentoring and professional development.

Requirements

      • Mandatory requirements are a relevant tertiary degree qualification and appropriate registration/licences.
      • A post graduate qualification or currently enrolled in a post graduate program in a relevant area of study is highly desirable.
      • The position involves travel by car or light aircraft to deliver outreach cardiac clinics and services across the communities within the North West Hospital and Health Service catchment. This includes visits to various facilities, community health centres, and remote units.
      • C Class Motor Vehicle Licence.
      • Pre-employment screening will be conducted for the successful applicant, including Criminal History checks.
      • This is a VPD risk role and may require evidence of vaccination or immunity to: Hepatitis A/B, Measles, Mumps, Rubella, Varicella, Pertussis, and Tuberculosis.
      • A period of probation may apply.
