About the role
The Nurse Navigator – Complex Care Coordinator is a registered nurse who demonstrates advanced levels in clinical skills, knowledge and translation of contemporary evidence-based care to practice. The Nurse Navigator aims to facilitate safe and efficient care for patients who are complex by reducing service fragmentation, enhancing management in partnership with the family and carers through understanding of their condition and coordinating the connection to appropriate services to enhance outcomes thus smoothing the patient journey.
About you
Our ideal candidate will be someone who can demonstrate the following:
- Advanced clinical evidence-based skill and knowledge in nursing, care coordination and interdisciplinary care management, evidenced by recency of practice and achieved clinical and professional competencies.
- Leadership proficiency through the articulation of team engagement principles and personal attributes that influence a positive team culture and achieve active collaboration across multidiscipline services.
- Proficiency in the establishment of therapeutic relationships, incorporating health education, advocacy and culturally competent professional practices that facilitate achieving patient identified goals and optimised health experiences.
- Demonstrated ability to collaborate at a work unit/service area level and/or across professions to ensure effective service delivery and quality outcomes.
Mandatory Requirements:
- Appointment to this position requires proof of qualification and registration or membership with the Australian Health Practitioner Regulation Agency (Ahpra).
- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
