About the role
You will provide advanced social work services to patients receiving renal care across North West Queensland.
- Deliver high-level social work services, psychosocial assessments, counselling, and care coordination for patients with chronic kidney disease and their families.
- Provide clinical leadership, supervision, mentoring, and professional support to staff and students while driving continuous service improvement.
- Advocate for patients and facilitate access to appropriate community, health, and support services across the care continuum.
- Partner with multidisciplinary teams to improve patient outcomes across Mount Isa and outreach locations including Cloncurry, Doomadgee, Normanton, and Mornington Island.
Working with us
- Eligible health practitioners entitled to accrue an additional three days professional development leave per annum, resulting in a total of six days professional development leave per annum.
- Eligible health practitioners working in designated remote areas will be entitled to a professional development allowance of $3,512.00 per annum.
- Rural and Remote incentive: Category D allowance of $431.22 p.f.
- Tax savings through salary sacrificing options are also available.
Requirements
- Potential applicants are advised that the Commission for Children and Young People and Child Guardian Act 2000 requires Queensland Health to seek a ‘working with children check; from the Commission for Children and Young People and Child Guardian prior to appointment to this position.
- Mandatory requirements are a relevant tertiary degree qualification and appropriate registration/licences/memberships.
- C Class Motor Vehicle Licence.
- Pre-employment screening will be conducted for the successful applicant, including Criminal History checks.
- This is a VPD risk role and may require evidence of vaccination or immunity to: Hepatitis A/B, Measles, Mumps, Rubella, Varicella, Pertussis, and Tuberculosis.
- A period of probation may apply.
