Your opportunity:
- To provide high level Social Work clinical services and professional expertise to patients on the Transition Care Program as part of the Integrated Care Rehabilitation Service.
- The position will focus on providing Social Work interventions for the rehabilitation of older people who have been discharged after an acute or sub-acute episode of care; preventing inappropriate and avoidable admissions to acute care or permanent residential aged care; and supervision of Allied Health Assistants in their implementation of delegated therapy programs.

Our offer:
You will receive a competitive remuneration package value up to $78,680 p.a., comprising salary rates $64.64 - $69.56 p.h., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HP4) (Two Permanent Part Time Positions, 38 hours p.f.).
