About us:

Cairns and Hinterland Hospital and Health Service provide an extensive range of health services at more than 30 regional, rural and remote facilities across a geographical area of 142,900 square kilometres. The Health Service is 95 percent self-sufficient with only a small number of high-level acute services being provided in Townsville and Brisbane.

The Advanced Health Worker is responsible for delivering culturally safe care at an advanced level to Hospital in the Home patients. The role focuses on health promotion, prevention, and the management of conditions, offering direct patient care within the home environment. Services are provided to inpatients across a community-based area, extending from Palm Cove in the north to Gordonvale in the south, and as far as the back of Lamb's Range to the west. Additionally, the Advanced Health Worker assumes full accountability and works collaboratively with a Registered Nurse or Clinical Nurse Consultant (CNC) to maintain high standards of care, supporting the Hospital in the Home team.

Our values were developed by our staff, patients and community. They guide our work and actions every day. Our staff are part of the community we serve. We believe that we can improve health outcomes by involving our community in the planning and evaluation of their local health services.

About your role:

  • Provide effective support and advocacy for identified Aboriginal and Torres Strait Islander Hospital in The Home clients by conducting timely home visits, case management conferencing, follow-up and referrals to relevant health services/programs.
  • Coordinate and conduct ongoing health promotion and education sessions with individuals and families within the local Aboriginal and Torres Strait Islander community. This may include but is not limited to, falls prevention, chronic disease, health screening, and general care for older people.
  • Empowering consumers to make informed health care decisions through consultation, education and information sharing.
  • Deliver evidence-based care by working collaboratively and in partnership with consumers, carers, external agencies and other health professionals.
