About us:

The Capricorn Coast Hospital and Health Service is within the Central Queensland Health Service, 38km or a short 40-minute drive from Rockhampton. Yeppoon is a sunny coastal town along the Capricorn Coast. The hospital integrates primary care with community care, all under one roof.

About the role:

The Clinical Nurse is accountable at an advanced level for the coordination of clinical practice delivered within the Hospital Avoidance and Sub Acute community program. The Clinical Nurse will be required to assess patients with complex care needs, identifying steps required to manage patients' ongoing health care needs after their initial encounter with the hospital.

  • You will provide advanced problem-solving skills, using well-developed clinical knowledge to deliver high quality, client orientated health service to clients with complex health and social situations.
  • As the Clinical Nurse, you will assist patients and their families with the transition from hospital to the home by supporting clients, their families and carers to access appropriate support from service providers within Aged Care or National Disability Insurance Scheme (NDIS).
  • You will assist clients and their families, with transition from hospital to Residential Aged Care facilities when living at home with service in place, is no longer a safe option.

About you:

  • Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA) and a current annual practicing certificate are mandatory for this position.
  • While not mandatory, a relevant qualification would be well regarded.

What we offer:

Total remuneration value of up to $65,949 p.a. is comprised of:

  • An hourly rate between $54.44 - $58.30 p.h.
  • 12.75% employer superannuation contribution
  • 17.5% leave loading
