What's involved – The Clinical Nurse Diabetes Educator role provides advanced clinical and problem-solving skills, expert planning, and coordination skills in the clinical management of patients/residents/clients with complex care needs, ensuring quality standards are met. The Diabetes Educator (DE) is expected to be able to practice autonomously and provide leadership in clinical decision-making for community-based diabetes education. They have the responsibility for providing professional direction and support to clinicians in this specialty practice area and to work collaboratively to lead practice changes and clinical innovation. The Diabetes Educator provides an integrated multidisciplinary coordinated approach to improve health outcomes for patients who have complex diabetes, usually with co-morbidities, providing strategies to reduce the risk of long-term complications and reducing the risk of hospitalisation.

As a member of the Primary Health Team, the role will work collaboratively and more broadly with the team and across Central West HHS to ensure continued focus on strategic priorities and delivery of operational imperatives.
