GEM-HITH provides comprehensive subacute care and you'll play a key role in assessments, clinical reviews, care planning and communication with families. About us:

Rockhampton Hospital is the major referral centre for Central Queensland, offering extensive specialist services across medicine, surgery, emergency, intensive care, paediatrics and more. Our GEMITH service supports older people experiencing age-related physical, functional or cognitive decline, delivering subacute care in their home to keep them connected, independent and well. We are a supportive, multidisciplinary team committed to delivering amazing care and building healthy communities.

About the role:

  • Deliver assessment, review and ongoing medical care for GEMITH patients.
  • Complete clear and comprehensive charting, documentation and discharge summaries.
  • Conduct offsite or telehealth reviews and attend scheduled clinics.
  • Assess suitability for GEMITH admissions and support day hospital reviews.
  • Communicate with patients, families and the MDT to coordinate care.
  • Work collaboratively with nursing and allied health to ensure safe, seamless care.
  • Manage acute or urgent presentations in consultation with senior medical staff.
  • Provide leadership in the absence of the Senior Medical Officer.

About you:

  • You hold (or are eligible for) AHPRA registration as a Medical Practitioner.
  • You bring sound clinical knowledge and PGY4+ experience.
  • You value teamwork, communication and patient-centred care.
  • You are confident working across home-based and hospital environments.
  • You demonstrate professionalism, ethical practice and clear clinical documentation.
  • You seek ongoing learning and contribute to quality improvement activities.
  • You hold current BLS certification and meet all mandatory vaccination requirements.

What we offer:

Total remuneration value of up to $147,585 p.a. (L7) or $176,992 p.a. (L9) is comprised of:

  • A salary between $134,489 - $155,125 p.a. (L4 - L9)
  • 12.75% employer superannuation contribution
  • 17.5% leave loading
