Your new role:

As the Clinical Nurse Consultant – Dementia & Delirium, you will provide advanced clinical leadership, specialist consultancy and expert assessment across Metro South. You will work closely with the Nurse Navigator, fellow CNCs, NUMs, and the Geriatrician to enhance patient outcomes and support high-quality, person-centred dementia care.

This role also plays a major part in supporting the Collaboratory assessment process and coordinating patient identification and transfer to the Mt Gravatt facility.

You will lead evidence-based practice, guide clinical decision-making, influence practice redesign, and contribute to a positive, education-focused culture.

Key Responsibilities

  • Provide expert clinical consultancy and advanced nursing practice for dementia and delirium across the care continuum.
  • Lead and model safe, high-quality, person-centred care aligned to NMBA standards and organisational frameworks.
  • Support nurses in care planning, delivery and evaluation, using advanced assessment and clinical reasoning skills.
  • Educate, mentor and influence multidisciplinary teams on contemporary dementia practice and research.

About you:

You are an experienced and highly skilled Registered Nurse with advanced expertise in dementia care, strong clinical leadership capability and a passion for innovation in person-centred practice. You thrive in collaborative environments, demonstrate critical thinking and are committed to improving outcomes for patients with dementia and delirium.

  • Extensive clinical knowledge and practical expertise in dementia nursing and managing Behavioural and Psychological Symptoms of Dementia.
  • Strong assessment, critical thinking and advanced clinical decision-making skills.
  • Experience developing detailed clinical reports, assessments and referral documentation.
  • Demonstrated ability to collaborate with internal and external stakeholders to support care transitions.

About us:

The Clinical Nurse Consultant (CNC) Specialist Dementia Care Program (SDCP) operates as part of the Princess Alexandra Hospital Dementia and Delirium Nursing Service. The CNC position is federally funded via the Commonwealth Specialist Dementia Care Program through provision of specialist clinical advice, care and support. The CNC establishes and maintains networks and partnerships with SDCP providers. The CNC position collaborates with a SDCP geriatrician in providing and facilitating pathways for SDCP residents through the Clinical Review Team.
