✈️ Permanent Position | Based in Cairns | Outreach to Southern Cape York Communities

Are you passionate about improving diabetes outcomes and empowering people to take control of their health? We are seeking a committed and highly skilled Clinical Nurse Consultant – Diabetes Educator to join our team in a permanent role based in beautiful Cairns, with regular outreach to southern Cape York communities.

🤝 About the role

This role will see you working collaboratively with the Director of Nursing of the Southern Primary Health Care Centre's in the development, delivery, and management of clinical services in remote TCHHS communities. You will provide advanced clinical expertise in diabetes management, education, prevention, health promotion, and care planning. The role includes leading quality improvement initiatives, contributing to service development, and working collaboratively with multidisciplinary teams and external stakeholders to improve health outcomes and reduce preventable hospital admissions.

  • Deliver advanced diabetes assessment, education, and management
  • Work closely with local health teams, GPs, and community health workers

  • Build capacity within local teams through mentoring and clinical guidance
