Are you passionate about improving diabetes outcomes and empowering people to take control of their health? We are seeking a committed and highly skilled Clinical Nurse Consultant – Diabetes Educator to join our team in a permanent role based in beautiful Cairns, with regular outreach to southern Cape York communities.
🤝 About the role
This role will see you working collaboratively with the Director of Nursing of the Southern Primary Health Care Centre's in the development, delivery, and management of clinical services in remote TCHHS communities. You will provide advanced clinical expertise in diabetes management, education, prevention, health promotion, and care planning. The role includes leading quality improvement initiatives, contributing to service development, and working collaboratively with multidisciplinary teams and external stakeholders to improve health outcomes and reduce preventable hospital admissions.

🏥 You will:
- Deliver advanced diabetes assessment, education, and management
- Work closely with local health teams, GPs, and community health workers
- Support culturally appropriate, patient-centred diabetes care
- Facilitate prevention, early intervention, and chronic disease management strategies
- Build capacity within local teams through mentoring and clinical guidance

- Registered Nurse with APHRA, with advanced practice skills
- Experience working in Aboriginal and Torres Strait Islander health (or a strong commitment to learning)

- Willingness to travel regularly for outreach

- Permanent full-time position
- Base salary comprising between $132,562.00 - $143,924.00 per annum – you may be eligible for additional benefits – refer to Workplace benefits | Torres and Cape Hospital and Health Service
