About the role
You will provide non-clinical home-based face to face assessments of clients referred via the My Aged Care Gateway using the Integrated Assessment Tool (IAT). Under the Single Aged Care Assessment System, Home Support Assessors will work with clients and their representative/s to establish goals specific to the client's needs and develops appropriate strategies and actions, including referrals to aged care and non-aged care services.
Credentialled aged care home support assessors will be highly valued.
The position requires you to use your own reliable vehicle and offers flexible work arrangements as you will be assessing older people in their homes.
About you
Our ideal candidate will be someone who can demonstrate the following:
- Achieve and maintain the required level of competency in Home Support assessment training provided by the Commonwealth, including the Gateway systems and the IAT and the wellness and reablement philosophy underpinning the assessment process.
- Ability to work under broad supervision and be able to undertake a diverse range of tasks as an effective member of a team in a high-pressure, high-volume work environment.
- Demonstrated communication skills, including accurate spelling, good comprehension, good verbal skills and the ability to deal courteously with people at all levels.
Mandatory Requirements:
- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
- Mandatory requirement of a Certificate III in Home Care or Aged Care or equivalent work experience
