Your opportunity:

  • The Geriatric Evaluation Management / Rehabilitation Hospital in The Home (GEM/GEMR HITH) is an initiative of the Wide Bay Hospital and Health Service funded through the Queensland Health Improvement Unit. The service is focused on improving health outcome for patients over the age of 65 years or 50 years for Aboriginal and Torres Strait Islander people, supporting avoidable admission and reducing length of stay in hospital for patients with age related physical, functional and /or cognitive decline associated with frailty.
  • The Clinical Nurse (GEM/GEMR HITH) demonstrates sound knowledge of contemporary nursing practice and theory and supports the Nurse Unit Manager and other members of the Integrated Care Services team timely transitions of care through the patient journey and the provision of clinical leadership, advice and advocacy to a multidisciplinary team.
  • This position provides expert clinical and patient-centred care within Integrated Care Services by supporting the facilitation and provision of quality and cost-effective nursing services which are consistent with contemporary standards and evidence-informed practice.

Temporary Full Time (76 hours p.f.) or Temporary Part Time Positions (hours negotiable) from date of appointment until 30th June 2026 with the possibility of extension

Our offer:

You will receive a competitive salary between $4,262.20 - $4,564.00 p.f., (f/t) or $56.08 - $60.05 p.h. (p/t), employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (Nurse Grade 6 (1)) (Temporary Full Time, 76 hours p.f., or Temporary Part Time Positions, hours negotiable, from date of appointment until 30th June 2026 with the possibility of extension.)
