Your opportunity:

  • The Transition Care Program (TCP) is a jointly funded initiative of the Commonwealth Department of Health and Ageing and Queensland Health, offering time-limited and goal oriented flexible rehabilitation care packages to older persons who have had a recent acute or sub-acute hospital admission and live within the Wide Bay Health Service region. It is aimed at older clients, but age alone is not an eligibility criteria.
    • Deliver effective rehabilitation occupational therapy clinical services to Transition Care clients:
    • maximises client function in activities of daily living,
    • maximises functional recovery
    • supports timely transition to the Transition Care program
    • supports clients' abilities to stay in their own home.

Our offer:

You will receive a competitive remuneration package value up to $132,567 p.a., comprising salary between $79,051 - $116,189 p.a., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HP3) (Permanent Full Time, 76 hours p.f.)
