This role is ideal for an experienced Nurse Practitioner who thrives on clinical autonomy, teamwork and meaningful community impact. You'll lead mobile, ED-substitutive care, respond rapidly to deterioration, and partner closely with GPs, aged care staff, families and multidisciplinary teams. Your expertise will not only improve resident outcomes but also build capability across aged care services through education, consultation and leadership.

About us:

Central Queensland Health delivers care across a diverse region, from regional centres to coastal and rural communities. Through the GERI RaSS program, we support residential aged care facilities with rapid geriatric assessment and intervention - reducing avoidable hospital admissions and strengthening care where people live.

About the role:

  • Provide advanced, autonomous geriatric and emergency nursing care in aged care settings.
  • Deliver rapid assessment, treatment and management of acute deterioration.
  • Lead and support a mobile ED-substitutive care team.
  • Work collaboratively under medical governance within a multidisciplinary model.
  • Educate and support aged care and senior nursing staff in gerontic care.
  • Contribute to service development, audits and quality improvement activities.

About you:

  • You are a registered Nurse Practitioner with current AHPRA endorsement.
  • You bring advanced clinical expertise in geriatric and/or emergency care.
  • You communicate confidently and build strong partnerships with teams and families.
  • You enjoy autonomy while valuing collaboration and shared decision-making.
  • You are committed to evidence-based practice, education and continuous improvement.

What we offer:

Total remuneration value of up to $15,261 p.m. is comprised of:

  • An hourly rate between $77.53 - $80.95 p.h.
  • 12.75% employer superannuation contribution
  • 17.5% leave loading
