About the Aged Care Assessment Team

The Gold Coast Aged Care Assessment Team (Single Assessment Service) is a community based, multidisciplinary team funded by the Australian Government. The role of the team is to work within the My Aged Care System to assess and support frail, older people to access the care they need to live healthy and safe lives in a place of their choosing. The team will soon be expanding our assessment role for older people entering the My Aged Care System and so we are seeking Home Support Assessors to join us in make a meaningful difference in the lives of our elderly community.

About Your New Role  

As a Home Support Assessor, you'll have a hands-on role, helping older adults remain safe and supported in their own homes. Working independently in clients' homes you will assess each client's functional abilities, identify needs, and create personalised, goal-focused care plans that encourage independence. If you're passionate about aged care, community-minded, and driven to make a difference, this role is ideal for you! You'll have the flexibility to travel across the Gold Coast Health and Metro South catchment areas, covering regions from west of Mount Tamborine up to Greenbank and down to the NSW border. Key Responsibilities

  • Undertake face to face care assessments in elderly persons home
  • Develop a client centred Support Plan with the client and or their representative
  • Identify clients who would benefit from a Reablement Focused Program
  • Review Reablement Focused Programs
  • Link clients and or representatives where required with key Aged Care Organisations

Qualifications and Experience

  • Certificate III in Home Care or Aged Care or equivalent work experience
  • Certificate IV in Health Service Assistance/Allied Health Assistance would be highly desirable
  • Experience in word processing, spreadsheet, database and email software, preferably Microsoft Office suite is highly desirable
  • Blue Card (GCH will cover cost)
  • Driver Licence
  • Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against - Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B
  • Tuberculosis screening, to identify if the tuberculosis vaccination may be required
  • Declaration of serious discipline history must be disclosed
  • Criminal History Screening will be required for new employees (GCH will cover cost)

Salary

  • Temporary full time (up to 30 June 2027 with the possibility of extension)
  • Base salary ranging from $3007.00 - $3341.00 per fortnight
