Your new role:
As a Nurse Navigator, you will provide advanced clinical leadership and coordination to support complex patient journeys, ensuring care is person-centred, culturally safe and seamlessly integrated across services.
- Deliver advanced autonomous clinical practice, coordinating patient-centred care across primary, secondary and tertiary settings
- Act as a central point of communication for patients, families and multidisciplinary stakeholders to reduce fragmentation and improve outcomes
- Apply expert clinical judgement and evidence-based practice to support safe, efficient and culturally responsive care pathways
- Contribute to system improvement through clinical governance, quality initiatives, education, research and service innovation
About you:
You are an experienced and motivated nursing professional who thrives in complex environments and is passionate about improving patient outcomes through coordination, leadership and collaboration.
- Registered Nurse with advanced clinical expertise and experience managing complex patient care across the continuum
- Demonstrated ability to work autonomously while leading and collaborating effectively with multidisciplinary teams
- Strong commitment to person-centred, culturally sensitive care that respects diversity, health literacy and patient empowerment
- Passion for clinical leadership, education, research and continuous improvement to enhance patient journeys and system efficiency

The Queen Elizabeth II Jubilee Hospital (QEII) is a medium metropolitan adult facility providing a range of inpatient and outpatient services including a 24-hour Emergency Department. We provide a range of services, including surgery, cardiology, endoscopy, orthopaedics, gynaecology, urology, geriatric, respiratory and emergency medicine.
We're making our hospital bigger and better to make it easier for our growing community to get the health care they need. We'll have more beds, medical wards and expanded intensive care and emergency services planned to be opened by 2027.
