About the role
Working within the inpatient setting, the Advanced Peer Worker will support consumers throughout their discharge journey to ensure planning is coordinated, person-centred, and reflective of individual needs, goals and strengths. This includes ensuring consumer voice is central to discharge planning and that the perspectives of family, carers and kin are included. The role collaborates closely with multidisciplinary teams to facilitate continuity of care and timely connections to NGOs, community services and other practical and psychosocial supports that strengthen recovery following discharge.
This position contributes to a service development project establishing a peer-led discharge support model within an acute inpatient setting and will assist in meeting project deliverables, key performance indicators and service evaluation or research activities. The successful candidate will bring strong knowledge of local services and lived experience of navigating complex systems to support individuals to overcome barriers, access appropriate supports, and build confidence as they transition back into the community.

Mandatory Requirements:
- A personal lived/living experience of Mental Health issues, and a personal experience in navigating Queensland Health Mental Health Acute and Community Services
- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
