What your day will look like….
Based in Toowoomba, you'll work with autonomy and purpose—supporting Aboriginal and Torres Strait Islander patients and families through flexible, person‑centred care planning, home visits, advocacy and community engagement. You'll bring strong cultural knowledge, compassion and integrity, demonstrating the Darling Downs Health values through respectful communication, collaborative practice and a genuine commitment to improving health outcomes for Aboriginal and Torres Strait Islander peoples. Alongside providing advanced clinical coordination, you'll help build meaningful community partnerships, mentor colleagues, contribute to health promotion and strengthen a culturally safe health service that honours identity, family and holistic wellbeing.
