About the role
As part of the headspace Clinical In‑reach Team (hCIT), you'll work across multiple headspace sites providing specialist assessment, intervention, consultation and shared care for young people experiencing complex mental health needs. This is a meaningful opportunity to support vulnerable young people and their families while contributing to service development within an innovative and highly collaborative model of care.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Deliver high-level and quality, evidence-based and social work / psychology / occupational therapy MHAOD assessment, intervention, and consultation for adolescents and young adults with complex mental health needs, including individual, group, and telehealth modalities.
- Develop and maintain effective collaborative relationships and communication with headspace staff, MNMH services (including Child and Youth, Adult, and Acute teams), adolescents and young adults, families and carers, multidisciplinary teams, general practitioners, and other government and non-government stakeholders to support integrated, coordinated, and individualised care for adolescents and young adults.
- Manage a clinical portfolio or additional team support functions as delegated by Team Leader, including monitoring and reporting on clinical practice and outcomes within the clinical service area to support achievement of key performance indicators.
