Your new role:
As a Senior Pharmacist within the GEMRHITH virtual ward, you will deliver expert clinical pharmacy services to older persons receiving hospital level care in their homes. Working across hospital and community environments (including Eight Mile Plains Community Health Centre), you will be a key member of a highly functioning multidisciplinary team.
You will:
- Deliver advanced, evidence-based clinical pharmacy services including medication assessment, intervention, review and discharge planning within a virtual inpatient model
- Conduct regular home visits within a defined catchment area to support safe, patient-centred medication management
- Provide clinical leadership, supervision and education to HP3 pharmacists, interns, students and Allied Health Assistants
- Lead and contribute to quality improvement, research, service development and evidence-based protocol advancement across geriatric medicine, pharmacy and community oral health services
About you:
You are an experienced clinical pharmacist with a passion for older persons' medicine and delivering care beyond traditional hospital walls.
You will demonstrate:
- High-level clinical expertise in geriatric pharmacotherapy and complex medication management
- Strong ability to prioritise, manage and discharge patients in a dynamic, high-acuity environment
- Advanced communication skills with the ability to influence, negotiate and collaborate across multidisciplinary teams
- Proven leadership capability in supervising, mentoring and developing less experienced clinicians and students

The Princess Alexandra Hospital (PAH) is a tertiary health care centre providing care in most major adult specialties. We provide world class health care to Brisbane City's southern suburbs across emergency, medical, mental health and surgical services, including liver and kidney transplant.
We are nationally recognised for our expertise in trauma management and organ transplants and are one of Australia's leading academic and research health centres. The PAH as a major medical research precinct hosts Australia's most advanced medical research facility, the Translational Research Institute (TRI) as well as a new Clinical Research Facility for the discoveries made at TRI to be trialed in a safe clinical environment.
