As a Clinical Nurse – Older Persons Health, you will provide advanced, autonomous care across gerontology and aged care assessment services. You will support comprehensive assessments, telehealth services, and care coordination to optimise independence and quality of life.

You will collaborate with multidisciplinary teams, apply evidence-based practice, exercise professional judgement within delegation frameworks, and contribute to service improvement, education, and quality initiatives across Community Health.

What's involved – The Clinical Nurse for Older Persons Health works autonomously to support patients through the use of advanced skills, knowledge, communication and experience in age care specific domains. 

This position assists across two domains; Gerontology and Aged Care Assessment. The Central West Health Gerontology Telehealth Service aims to optimise the functioning and independence of older people with complex needs associated or related to ageing emphasising health promotion, disease prevention and the management of chronic conditions, enhancing quality of life. The Aged Care Assessment -QLD Health Single Assessment CW (ACA) have been established throughout Australia by the Australian Federal Government to deliver comprehensive assessment and care coordination for frail and older persons to assist in gaining access to the most appropriate care. To meet program delivery goals and objectives, the ACA has the responsibility to conduct assessment in accordance with the Aged Care Legislation and Age Care Act, 2025.
