The CHQ at Home Support Worker is responsible for the support of a child or young person being cared for within their home environment. The support worker will be trained and supported to provide safe care to a child or young person with high medical needs within their home or community environment under the supervision of the primary caregiver.

About the Team
The CHQatHome service has multiple programs aimed at supporting children to receive appropriate care closer to home.
The service functions as a multidisciplinary team, including nursing, medical, physiotherapy, social work, pharmacy, psychology, dietetics, occupational therapy, speech pathology, psychology, administration and operational officers.
CHQatHome currently supports the following programs:
- Hospital in the (HITH)
- Post-Acute Care (PAC)
- Hospital Avoidance (HA)
- Home Ventilation Program (HVP)
- Tracheostomy in Home Care (TIHC)
- Support of Palliative Care Service
The role of the Support Worker is specifically aimed to support the Home Ventilation Program and Tracheostomy in Home Care Program.
About the Role
You will fulfil the accountabilities of this role in accordance with the CHQ HHS core values and as outlined below.
- Assisting the child's primary caregiver to maintain an environment conducive to meeting the child's health care needs.
- Responsible for fulfilling tasks delegated by a primary care giver in alignment with CHQ at Home team training.
- Tasks may include but are not limited to:
- Assisting the primary care giver in the support of a child or young person with additional needs such as tracheostomy care, ventilation support/management, and gastrostomy care.
- Maintaining a clean and safe environment for the child (e.g. clean suction and medical equipment).
- Notifying the primary caregiver of any change in the condition of the child as per CHQ at Home training.
- Assisting the primary caregiver with the child's feeding, hygiene cares and routines as required including the provision safe manual handling practices.
