Role details
The Aboriginal and Torres Strait Islander Health Unit at the Sarina Hospital is seeking applications for a Clinical Nurse (Potentially Preventable Hospitalisations Team) to join their team on a permanent basis.

Your opportunity
The Clinical Nurse (Potentially Preventable Hospitalisation) is an employee who is a registered nurse who:
- Is appointed to an advanced practice nursing position; and
- Demonstrates a specialised clinical expertise chronic disease specialised skill set.
- Promotes and ensures the provision of best practice nursing care in the assessment and treatment of First Nations people living with, or at risk of a chronic condition to maintain and improve their health, functional ability and quality of life in the community.
- Lead in identifying and addressing Potentially Preventable Hospitalisations to meet the Key Performance Indicator under the Closing the Gap Initiatives.
- Provides expert clinical nursing, to ensure the full range of clinical care, prevention, promotion and early intervention activities are culturally appropriate and lead to improved health outcomes and access to primary health care services for First Nations people.
- Delivers outreach services to First Nations patients who are at risk of or currently have a chronic condition within Mackay/Sarina area.
About the Team
The Aboriginal and Torres Strait Islander Health Unit is an essential element of the MHHS support services and ensures the development and delivery of culturally appropriate services and models of health care to Aboriginal and Torres Strait Islander people and families.
