We're excited to welcome a Clinical Nurse to our Transition Care Program.
The Transition Care Program is a national program which provides short term support and active management for older persons after an acute or subacute episode with an aim to increase the person's level of function through goal orientated care that has a therapeutic approach.
Key Responsibilities include:
- providing nursing and/or midwifery services in health service settings
- responsible for a portfolio with a focus on clinical care/leadership, management, education and/or research
- The activities required of roles at this level are predominantly clinical in nature but also provide support to a Nurse Grade 7 or above in management activities
Remuneration value up to $135 857 p.a., comprising salary between $111,198 - $119,072 p.a. (f/t), or salary rates: $56.08 - $60.05 p.h. (p/t), employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (Nurse Grade 6 (1) (full time and part time positions available, hours negoitable).
