About the role
As a Clinical Nurse (Nurse Grade 6 Band 1), you will practice autonomously while providing expert clinical leadership and decision‑making within the Brighton Rehabilitation Unit. The role is predominantly clinical, with a strong focus on discharge coordination, patient flow, and complex care planning, while also supporting management, education, and quality improvement activities.
You will deliver advanced nursing care to patients with a range of orthopaedic, neurological, multi‑trauma and chronic conditions, ensuring discharge planning commences at admission and is integrated throughout the patient journey. Acting as a key link between patients, carers, community services and the interprofessional healthcare team, you will identify barriers to discharge and facilitate timely, safe and effective transitions of care.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Practice in accordance with code of ethics, professional standards and legislation affecting nursing practice by maintaining current knowledge and competence within scope of position.
- Apply advanced clinical nursing expertise, including assessment, planning, implementation, and evaluation, to deliver a high standard of patient centred care.
- Respond appropriately to and provide support and direction to manage sudden alterations in the health status of the patient using advanced nursing assessment and intervention skills.
- Support patient flow and timely patient discharge by demonstrating comprehensive care planning processes, including goal setting and discharge planning from admission in collaboration with the interprofessional team.
