About the role
It is the purpose of this role to provide culturally sensitive social, emotional, and spiritual well-being support to Aboriginal and Torres Strait Islander people with a mental illness, including direct care coordination and support to the consumer, their families and community. This also includes working as a member of a mental health multidisciplinary team in the provision of triage, assessment, transitional planning and co-case management for people accessing mental health services, and advice to staff on social and cultural aspects of assessment, treatment, and recovery.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Ability to work effectively and credibly with Aboriginal and Torres Strait Islander people (including clients, carers, and Metro North Health staff) with a clear understanding of Aboriginal and Torres Strait Islander culture, protocols, and implementation of culturally safe practice.
- Utilises cultural assessments and interventions to contribute to mental health early intervention and relapse prevention strategies for Aboriginal and Torres Strait Islander consumers, including brief interventions to inform mental health and social and emotional wellbeing care and transitional care pathways.
- Contribute to the understanding of individual consumer presentations by facilitating culturally appropriate assessment, interventions and transitional planning and assisting with interpretation of information from a cultural perspective, including attendance at Emergency Departments/Psychiatric Emergency Centre, inpatient, home visits and community locations and providing co-case management.
