Role details

The Continuing Care Team at the Mackay Community Health Centre Seeking applications from Occupational Therapist or Psychologist or Social Worker or Clinical Nurse to join their team on a permanent basis.

Your opportunity

  • The successful post holder will function as a Mental Health Clinical Nurse as part of a multidisciplinary team responsible for coordinating and delivering case management and care coordination activities. The Clinical Nurse will manage a case load that supports individuals with serious mental illness to provide specialist assessment, crisis management, care coordination, delivery of nursing interventions, case review, advanced care planning and risk management to the Continuing Care Team.
  • This role will also contribute to the planning, development, implementation, maintenance and evaluation of a broad range of treatment and prevention activities to promote recovery from mental illness.

About the Team

The Continuing Care Team (CCT) works individually and in group settings to provide ongoing mental health interventions. The service objectives are:

  • Always strive to have the consumer and family or carers at the centre of any care intervention.
  • To provide holistic, contemporary recovery focussed mental health care.
  • To promote and maintain mental and physical wellbeing for all consumers.
  • To ensure that services are accessible and responsive to individual needs.
  • To ensure services are delivered safely, reducing risk or potential harm as much as possible.
  • To ensure that any intervention assists the individual maintain an engaging and self-determined lifestyle in the community.
  • Ensure the service is evaluated via accreditation, KPI's and other methods of audit or feedback informing continuous service improvement.
  • To have a capable workforce able to respond to the complex competing needs of the population we serve.
  • To advocate for the consumer who may be experiencing stigma or marginalisation as a result of mental illness and psychiatric disability.
  • To educate and raise awareness of serious mental illness to the wider community.
  • To ensure the service delivered is value for money.
