The Diabetes Service delivers nurse led diabetes education clinics (adult and paediatric) and contributes to a broader diabetes multidisciplinary medical and allied health team to provide expert, quality, accountable care, and education in diabetes to patients across the care continuum and in accordance with prescribed professional and ethical standards and evidence-based practice. This role provides a central point of contact, for clinical consultation, and referral for medical, nursing, and allied health staff.

About the role
The Clinical Nurse Consultant (CNC) Diabetes is an advanced practice Registered Nurse who provides clinical leadership to the West Moreton Health Diabetes Service by coordinating and delivering specialist clinical services across the care continuum, including acute inpatient services, outpatient department and antenatal clinics, within the specialty area of diabetes education and management.
About you
Our ideal candidate will be someone who can demonstrate the following:
- Demonstrated advanced clinical knowledge in assessment, education, and management for patients (adult and paediatric) living with diabetes including contemporary insulin systems (MDI and CSII) and skills in flash and CGMS initiation and data interpretation.
- Additional consideration will be given to applicants who can demonstrate experience in providing evidence-based self-management education and care activities for children and young people and their families diagnosed with diabetes (of any type including any insulin requirement).
- Advanced clinical skills in the identification and adoption of innovative clinical practice models and work practices that develop partnerships to support the continuum of care including contemporary technologies (IT, ICT).
Mandatory Requirements:
- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
