Your new role:
This is your opportunity to take the lead in coordinating complex care for children and young people experiencing acute mental health challenges. As a Nurse Navigator, you'll drive seamless care, reduce system barriers, and improve outcomes across the continuum.
In this role, you will:
- Lead end-to-end care coordination across hospital and community services
- Deliver brief, targeted interventions to young people and families in crisis
- Streamline pathways and reduce fragmentation across the care journey
- Partner with multidisciplinary teams and external stakeholders
- Champion service innovation and system improvement initiatives
About you:
You are a confident, driven mental health nurse who thrives in complex environments and is passionate about improving care for young people.
You will bring:
- Advanced experience in child and youth mental health settings
- Proven ability to lead, influence and coordinate complex care
- Strong communication skills with families, teams and stakeholders
- Systems thinking with a focus on improving patient journeys
