About us:
The Pharmacy Service provides medication management across the Healthcare facilities CHHHS. The services provides full and extensive Pharmacy Services at a CSCF Level 5. The Clinical Services Capability Framework (CSCF) is Queensland Health's statewide framework that defines the minimum requirements (workforce, support services, safety, and governance) for health services to safely deliver care at different levels of clinical complexity.
This position reports to the Pharmacist Advanced – Community Services Clinical Lead and Assistant Director of Pharmacy Rural and Community.
About your opportunity:
- Deliver high level clinical pharmacy services within the Community Care team within the CHHHS in accordance with prescribed professional and ethical standards with infrequent need for direct clinical practice supervision.
- Participate in a structured mentoring program for pharmacists including community providers to develop specialist skills within the rural and community service for patients' self-identifying as First Nations and for Rural and Remote patients.
- Provide general clinical advice to professional and operational supervisors, relevant service managers and other stakeholders regarding clinical pharmacy services delivery.
- Liaise with patients and their families, other health professionals, supervisors, students and researchers, as required to ensure continuum of care.
- Monitor and report on clinical practices and outcomes within clinical service area and actively participate in initiating, developing and implementing quality and service improvement activities that enhance the delivery of clinical pharmacy services.
