Your new role:
As an advanced practice clinician, you will lead the delivery of comprehensive, evidence-based geriatric care within a collaborative multidisciplinary environment by:
- Providing advanced assessment, diagnosis, treatment and care coordination for older patients with complex conditions
- Leading nurse-led models of care to improve patient outcomes and system efficiency
- Working across inpatient and outpatient settings within the Acute Care of the Elderly service
- Prescribing, interpreting diagnostics and implementing evidence-based interventions within scope of practice
- Partnering with multidisciplinary teams to deliver integrated, person-centred care
- Driving quality improvement, service innovation and clinical excellence in geriatric care
- Supporting care navigation, referrals and continuity across hospital and community settings
About you:
You are a highly skilled and autonomous Nurse Practitioner with a passion for improving outcomes for older persons, bringing:
- NMBA endorsement as a Nurse Practitioner with a Master's qualification (mandatory)
- Advanced clinical expertise in geriatrics, including frailty, cognitive impairment and complex comorbidities
- Proven ability to deliver comprehensive assessments, prescribe and manage patient care independently
- Strong leadership capability with experience guiding multidisciplinary teams and driving service improvements
- Demonstrated commitment to evidence-based practice, research and continuous quality improvement
- Excellent communication, collaboration and stakeholder engagement skills
- A patient-centred approach with a focus on innovation, safety and quality outcomes
