Your opportunity
The HOPE Program Manager will provide advanced expertise, strategic leadership and innovation, and operational management of the multidisciplinary HOPE team. The HOPE Program Manager will also provide support and guidance to the Care Collective Community Care Service Coordinator/s under The Care Collective - South West Primary Care Pilot to ensure synergy in service provision. More specifically, the role is responsible for the multi-location coordination, implementation and evaluation of the HOPE model of service delivery. The Program Manager will support strong integration within the communities it services and supports, across the 3 hubs (Roma, Charleville and St George) and 15 smaller communities of South West Hospital and Health Service.
The HOPE Program Manager is a member of the SWHHS Senior Leadership Team and reports directly to the Executive Director of Allied Health.
This position is responsible for coordination, financial delegation and direct management responsibility of the HOPE team members located in Charleville, Roma and St George.
